Private Controlled Drug (CD) Prescribers

The Private Controlled Drug (CD) Prescriber code referred to in the following procedures, is a 6 figure code which is allocated by the PPD.
Prescriptions issued by private CD prescribers are processed by the PPD using the allocated code.

The PCT or Agency working on behalf of the PCT will advise the PPD, of any new or changes to existing private CD prescribers, by completing the Notification of Private CD Prescriber Amendments Form Microsoft Word document.

The following information must be completed on each form:

  • If the form was submitted by a PCT, the PCT name and PCT code
  • If the form is submitted by an Agency, the Agency name, PCT name and PCT code
  • Profession – Tick the appropriate box
  • Signature of authorized signatory
  • Date the amendment completed
  • Name of Signatory printed
  • Telephone number of authorized signatory – in case of query

In order to avoid transposition errors and the subsequent problems incurred, it may be beneficial to complete the forms electronically.

Private CD Prescriber becoming responsible to the PCT

The PCT will advise the PPD by completing the Notification of Private CD Prescriber Amendments Form Microsoft Word document.

The following information must be completed

Update Type

Tick the New Prescriber box.

Prescriber Code

If the private CD prescriber is also working in the NHS primary care sector then please advise of the GP National code or, for all other prescribers, the regulatory body code.

SECTION A: Prescriber Details

  • Prescriber Surname and Initials
  • Title (Mr/Mrs/Miss/Ms/Sister)
  • Qualified to prescribe -Tick if qualified to prescribe from DN/HV formulary or extended formulary/supplementary prescriber (applicable to private CD nurse prescriber only).

SECTION B: Contact Details

  • Contact address including Post Code
  • Contact Telephone number.

SECTION C: Effective date

  • The start date of the prescriber as a private CD prescriber.
Following a request for prescription forms from the private CD prescriber the PCT will submit an order to 3MSPSL, using the 3MSPSL ordering procedure.

Private CD prescriber no longer responsible to the PCT

The PCT will advise the PPD by completing the Notification of Private CD Prescriber Amendments Form Microsoft Word document.

The following information must be completed

Update Type

Tick the Prescriber no longer the responsibility of the PCT box.

Prescriber Code

  • Private CD Prescriber Code

    and
  • If the private CD prescriber is also working in the NHS primary care sector then please advise of the GP National code or for all other prescribers the regulatory body code.

SECTION A: Prescriber Details

  • Prescriber Surname and Initials
  • Title (Mr/Mrs/Miss/Ms/Sister)
  • Qualified to prescribe -Tick if qualified to prescribe from DN/HV formulary or extended formulary/supplementary prescriber (applicable to private CD nurse prescriber only).

SECTION B: Contact Details

  • Contact address including Post Code
  • Contact Telephone number.

SECTION C: Effective date

  • The end date of the prescriber as a private CD prescriber.
The PCT must ensure that all unused FP10CDNC and FP10CDSS prescription forms are collected and destroyed in a secure manner

Change of Surname

The PCT will advise the PPD by completing the Notification of Private CD Prescriber Amendments Form Microsoft Word document.

The following information must be completed

Update Type

Tick Change of Surname box.

Prescriber Code

  • Private CD Prescriber Code

    and
  • If the private CD prescriber is also working in the NHS primary care sector then please advise of the GP National code or for all other prescribers the regulatory body code.

SECTION A: Prescriber Details

  • Prescriber Surname and Initials – new Surname
  • Title (Mr/Mrs/Miss/Ms/Sister)
  • Qualified to prescribe -Tick if qualified to prescribe from DN/HV formulary or extended formulary/supplementary prescriber (applicable to private CD nurse prescriber only).

SECTION B: Contact Details

  • Contact address including Post Code
  • Contact Telephone number.

SECTION C: Effective date

  • The effective start date of the new Surname.
The PCT must:

  • Order personalised prescription pads from 3MSPSL, in respect of the private CD prescriber’s new surname, using the 3MSPSL ordering procedure, following a request from the private CD prescriber.
  • Ensure that all unused FP10CDNC prescription forms bearing the previous surname are collected and destroyed in a secure manner

Change of Telephone Number

The PCT will advise the PPD by completing the Notification of Private CD Prescriber Amendments Form Microsoft Word document.

The following information must be completed

Update Type

Tick change of Telephone Number box.

Prescriber Code

  • Private CD Prescriber Code

    and

  • If the private CD prescriber is also working in the NHS primary care sector then please advise of the GP National code or for all other prescribers the regulatory body code.

SECTION A: Prescriber Details

  • Prescriber Surname and Initials
  • Title (Mr/Mrs/Miss/Ms/Sister)
  • Qualified to prescribe -Tick if qualified to prescribe from DN/HV formulary or extended formulary/supplementary prescriber (applicable to private CD nurse prescriber only).

SECTION B: Contact Details

  • Contact address including Post Code
  • Contact Telephone number – new Telephone number.

SECTION C: Effective date

  • The start date of the new Telephone number.

The PCT must:

  • Order personalised prescription pads from 3MSPSL, in respect of the private CD prescriber’s new telephone number, using the 3MSPSL ordering procedure, following a request from the private CD prescriber.
  • Ensure that all unused FP10CDNC prescription pads bearing the previous telephone number are collected and destroyed in a secure manner

  • The start date of the new Title.

The PCT must:

  • Order personalised prescription pads from 3MSPSL, in respect of the private CD prescriber’s new title, using the 3MSPSL ordering procedure, following a request from the private CD prescriber
  • Ensure that all unused FP10CDNC prescription pads bearing the previous title are collected and destroyed in a secure manner.

  • Ensure that all unused FP10CDNC prescription pads bearing the previous address/ post code are collected and destroyed in a secure manner.
