Controlled Drug (CD) Prescribers
The
Private Controlled Drug (CD) Prescriber code referred
to in the following procedures, is a 6 figure code which
is allocated by the PPD.
Prescriptions issued by private CD prescribers are processed
by the PPD using the allocated code.
The
PCT or Agency working on behalf of the PCT will advise
the PPD, of any new or changes to existing private CD
prescribers, by completing the Notification
of Private CD Prescriber Amendments Form .
The
following information must be completed on each form:
- If
the form was submitted by a PCT, the PCT name and
PCT code

If the form is submitted by an Agency, the Agency
name, PCT name and PCT code

- Signature
of authorized signatory
