Contractors (Pharmacy and Appliance)

The OCS code which is referred to in the following procedures is a code which is supplied by the PPD to each contractor. The information must be supplied to the PPD in a timely manner.

Local Pharmaceutical Services (LPS) Pilots are being introduced to demonstrate new ways in which to organise and pay for community pharmacy to deliver a wider range of services than under the current national contract, enabling local needs to be met more effectively.

The PCT will advise the PPD of a change to a contractor by completing the relevant proforma.

The following details must be completed on each form:

  • If the form was submitted by an Agency, the Agency name, PCT name and PCT code
  • If the form was submitted by a PCT, the PCT name and PCT code
  • Indicate if the change is in respect of a pharmacy or appliance contractor
  • Signature of the authorised signatory
  • Telephone number of authorised signatory
  • Name of signatory printed
  • Date the amendment was completed
  • Position of Signatory.

NB Please inform your nhs.uk web editor of any changes notified to the PPD.
