General
Practitioner (GP)
The GP's
National Code Number (NCN) which is referred to in the following
procedures is a 6 figure numeric code which is supplied by
the Department of Health to a doctor when they first register
as a GP. This code is unique and will move with the GP from
practice to practice.
Prescriptions
are processed by the PPD and ultimately prescribing costs
are allocated to the prescriber's budgets using the 6 figure
NCN printed on their prescription forms.
The PCT
will advise the PPD of a change to a GP/Practice details by
completing the relevant proforma
(or inform in letter). The following details must be completed
on each form:
- If
the form was submitted by an Agency, the Agency name, PCT
name and PCT code
- If
the form was submitted by a PCT, the PCT name and PCT code
- Signature
of the authorised signatory
- Telephone
number of authorised signatory in - case of query
- Name
of signatory printed
- Date
the amendment completed.
