Hospitals

The hospital unit code which is referred to in the following procedures is allocated by:

  • The PCT and advised to the PPD

These procedures only relate to hospitals units that issue hospital prescriptions that will be dispensed in the community.

Hospital Prescription forms dispensed in the community are processed by the PPD and ultimately recharged to the PCT using the hospital code identified on the Forms FP10(NC), FP10SS, and FP10 MDA-SS.

The PCT will advise the PPD of a change to hospital details by completing the relevant proforma. The following details must be completed on each form:

  • If the form was submitted by an Agency, the Agency name, PCT name and PCT code
  • If the form was submitted by a PCT, the PCT name and PCT code
  • Signature of authorised signatory
  • Name of signatory printed
  • Telephone number of signatory
  • Date the amendment was completed.
