  Report Recipient

The PPA can only provide Prescribing information to PCT Authorised Personnel (Report Recipient).

The PCT will advise the PPA of a change to a report recipient by completing the relevant proforma with the following information:

  • The PCT name
  • The PCT code
  • Signature of Report Recipient
  • Date the amendment completed
  • Telephone Number of Report Recipient
  • Signature of Prescribing Lead
  • Date the amendment completed
  • Telephone Number of Prescribing Lead.

  3. Change of Report Recipient Surname
  4. Change of Report Recipient Address
