The PPA
can only provide Prescribing information to PCT Authorised
Personnel (Report Recipient).
The PCT will advise the PPA of a change to a report recipient
by completing the relevant proforma with the following information:
- The
PCT name
- The
PCT code
- Signature
of Report Recipient
- Date
the amendment completed
- Telephone
Number of Report Recipient
- Signature
of Prescribing Lead
- Date
the amendment completed
- Telephone
Number of Prescribing Lead.
