Dear Sirs, Re: Freedom of Information Act and Part IXA of the Drug Tariff Referring to Part IX A of the Drug Tariff, I would like to request data/information on the following medical devices, which have been approved and listed in the Tariff: A) Irrigation Fluids 1) Irriclens (ConvaTech)Aerosol 1x240ml 2) MiniVersol (Aguettant Ltd) Bottle (45ml & 100ml) 3) Nine Lives (Nine Lives) Can (1x200ml) 4) Normasol (Seton) Sachet (25ml & 100ml) 5) Stericlens (C D Medical Ltd) Aerosol (100ml & 240ml) 6) Tubilux Nasal Drops (Tubilux Pharma) Dropper bottle (1x10ml) B) Nasal Device 1) Nozovent (Anglian Pharma) Nasal Dilator to reduce snoring The data/information I would like to request on each of the above stated devices is a copy/transcript of - application forms DT1 submitted by the applicants - information/data/evidence submitted supporting the appropriateness for GP & nurse prescribing - information/data/evidence submitted proving their cost effectiveness - Any independent information/advice sought by the PPA in relation to the approval of those devices and the replies to those by the independent advisors - any information/notes/comments made by the Pharmaceutical Services Manager with regards to the approval of any of the above devices
