Claims for Fees relating to the Decommissioning of Authorised Headsets held by a Pharmacy Contractor

Pharmacy contractors who provided an oxygen cylinder service on or after 30 September 2005 may submit a claim to the local PCT, for a one-off payment for the withdrawal from service (or decommissioning) of authorised headsets purchased by the pharmacy contractor. Claims should be submitted (by post or e-mail) to the local PCT on or before 31 December 2006 using the claim form below.
