The PCT will advise the PPA of a Contractor joining the PCT by:

• Completing the proforma - Notification of Pharmacy/Appliance Contractor Amendments (PPA 305) click here to print copy.
  
• Advising the PPA of the banking details by using the proforma - Notification of Contractor's Bank & Correspondence Details (PPA 302).

Tick the box - Addition

The Addition column should be completed with the following details:

• Owner/Company name
  
• Trading name (if different to owner name)
  
• Address (premises address should be completed)
  
• Postcode (postcode of premises)
  
• Telephone Number (telephone number of premises)
  
• Tick if LPS Contract
  
• The effective start date.

The PPA will advise the PCT/Agency of the contractor's OCS code by letter.
