The PCT should advise the PPA of a change in status for Payment for additional professional services by using the proforma - Notification of Pharmacy/Appliance Contractor Amendments(PPA 305) click here to print copy.

Tick the box - Change of Additional Professional Services Status.

The Addition column should be completed with the following details:

• OCS Code
  
• Owner/Company name
  
• Trading name (if different to the owner name)
  
• Address of the contractor (premises address should be completed)
  
• Postcode (postcode of premises)
  
• Telephone Number (telephone number of premises)
  
• Tick if eligible for payment for additional professional services (further information can be found in the Drug Tariff)
  
• Tick if LPS Contract
  
• Tick if Repeat Dispensing Contract
  
• The effective start date - as the PPA can only pay full months this date must be the start of a month.
