Tick the box - Change of PCT.

Two forms must be supplied:

• One by the PCT the contractor is leaving
  
• One by the PCT the contractor is joining.

NB Both forms must be received before the PPA can action the amendment.

The Deletion column should be completed with the following details, by the PCT the contractor is leaving. Click here for instructions.

The Addition column should be completed by the PCT with the following details, by the PCT the contractor is joining:

• OCS code
  
• Owner/Company name
  
• Trading name (if different to owner name)
  
• Address (premises address should be completed)
  
• Postcode (postcode of premises)
  
• Telephone number (telephone number of premises)
  
• Tick if LPS Contract
  
• The effective start date.
