Change of Ownership

The PCT will advise the PPA of a transfer of ownership by:

• Completing the proforma - Notification of Pharmacy/Appliance Contractor Amendments (PPA 305) click here to print copy.
  
• Advising the PPA of any changes to the banking details using the proforma - Notification of Contractor's Bank & Correspondence Details (PPA 302).

Transfer of Ownership when the Bank Account has not been purchased

If a transfer of ownership occurs where a company has bought a business but not the Bank Account then the PPA will issue a new OCS code to the contractor.

The PPA will advise the PCT of the contractor's OCS code by letter.

The Deletion column should be completed with the following details in respect of the existing contractor. Click here for instructions.

The Addition column should be completed with the following details in respect of the new owner:

• Owner/Company name
  
• Trading name (if different to the owner name)
  
• Address (premises address should be completed)
  
• Postcode (postcode of premises)
  
• Telephone number (telephone number of premises)
  
• Tick if LPS Contract
  
• The effective start date.

Transfer of Ownership when the Bank Account has been purchased

If a transfer of ownership occurs where a company has bought a business and the Bank Account then the contractor will retain the existing OCS code.

The Addition column should be completed with the following details in respect of the new owner:

• OCS code
  
• Owner/Company name
  
• Trading name (if different to the owner name)
  
• Address (premises address should be completed)
  
• Postcode (postcode of premises)
  
• Telephone number (telephone number of premises)
  
• Tick if eligible for payment for additional professional services (further information can be found in the Drug Tariff)
  
• Tick if LPS Contract
  
• Tick if Repeat Dispensing Contract
  
• The effective start date.
