The PCT should advise the PPA of a change in eligibility for Payment for additional professional services by using the proforma - Notification of Pharmacy/Appliance Contractor Amendments (PPA 305) click here to print copy.

The Addition/Amendment column should be completed with the following details:

• Circle amendment
  
• Owner name (company or individual)
  
• Trading name (if different to the owner name)
  
• Address of the contractor (premises address should be completed)
  
• Postcode (postcode of premises)
  
• Telephone Number (telephone number of premises)
  
• Tick if eligible for Payment for additional professional services (further information can be found in the Drug Tarrif)
  
• Start date - as the PPA can only pay full months this date must be the start of a month.
