The PCT will advise the PPA of a transfer of ownership by:

• Completing the proforma - Notification of Pharmacy/Appliance Contractor Amendments (PPA 305) click here to print copy.

• Advising the PPA of any changes to the banking details using the proforma - Notification of Contractor's Bank & Correspondence Details (PPA 302).
  

Transfer of Ownership on a non debts and liabilities basis

If a transfer of ownership occurs where a company has bought a business on a non debts and liabilities basis the PPA will issue a new OCS code to the contractor.

The Addition/Amendment column should be completed click here for instructions.

The PPA will advise the PCT of the contractor's OCS code by letter.

Transfer of Ownership on a debts and liabilities basis

If a transfer of ownership occurs where a company has bought a business and takes on debts and liabilities the contractor will retain the existing OCS code.

The Deletion/Previous/Relocation From column should be completed with the following details in respect of the existing contractor:

• Circle previous
  
• OCS code
  
• Owner name (company or individual)
  
• Trading name (if different to the owner name)
  
• Address (premises address should be completed)
  
• Postcode (postcode of premises)
  
• Telephone number (telephone number of premises).

The Addition/Amendment column should be completed with the following details in respect of the new owner:

• Circle amendment
  
• OCS code
  
• Owner name (company or individual)
  
• Trading name (if different to the owner name)
  
• Address (premises address should be completed)
  
• Postcode (postcode of premises)
  
• Telephone number (telephone number of premises)
  
• Indicate if eligible for Payment for additional professional services (further information can be found in the Drug Tariff)
  
• Start date.
