PCT Details
The PPD hold PCT details. The PCT should advise the PPD of changes to:
- PCT Address
- PCT Telephone Number
1. PCT Address
The PCT should advise the PPD of a change of address using headed notepaper and must be signed by a PCT prescribing lead.
The PCT should identify the new address and the effective start date of the new address.
2. PCT Telephone Number
The PCT should advise the PPD of a change of telephone number using headed notepaper and must be signed by a PCT prescribing lead.
The PCT should identify the new telephone number and the effective start date of the new telephone number.
