Change of Address

The PCT will advise the PPA of a change of address for a GP or practice by using the proforma - Notification of GP/Practice Amendments click here to print copy. One form per GP/Practice should be completed.

Tick the box - Change of Address.

The Deletion column should be completed click here for instructions.

The Addition column should be completed with the following details:

• New address of GP/Practice
  
• Postcode (the postcode of the surgery identified in the address box)
  
• Telephone number
  
• The effective start date.

The PCT must:

• Order personalised prescription pads from Astron in respect of the new address using the Astron ordering procedure
  
• Ensure that all unused FP10 prescription pads bearing the previous address are collected and destroyed in a secure manner
  
• Ensure that the practice amend their software to ensure that no further prescriptions are issued bearing the GP's previous address.
