The PCT will advise the PPA of any other changes by using the proforma - Notification of GP/Practice Amendments click here to print copy.
Eg. Changing from prescribing to dispensing, from partner to salaried GP or from limited service to full GMS.

Tick the box - Change of Other Details.

The Deletion column should be completed click here for instructions.

The Addition column should be completed with the following details:

• The new status
  
• The effective start date.
