Change of PCT

The PCT will advise the PPA when a practice changes their responsible PCT by using the proforma - Notification of GP/Practice Amendments click here to print copy.

Tick the box - Change of PCT.

Two forms must be supplied:

• One by the PCT the practice is leaving
  
• One by the PCT the practice is joining.

NB Both forms must be received before the PPA can action the amendment.

The Deletion column should be completed with the following details, by the PCT the practice is leaving:

• Partnership - the name of the senior partner
  
• Address (the address of the main surgery)
  
• Postcode (the postcode of the surgery identified in the address box)
  
• Telephone number (the telephone number of the surgery including the STD code identified in the address box)
  
• Tick if the practice is a PMS pilot site
  
• The responsible payment site of the practice (your Finance Department may be able to advise)
  
• The effective end date.

The Addition column should be completed with the following details, by the PCT the practice is joining:

• Partnership - the name of the senior partner
  
• Address (the address of the main surgery)
  
• Postcode (the postcode of the surgery identified in the address box)
  
• Telephone number (the telephone number of the surgery identified in the address box)
  
• Tick if the practice is a PMS pilot site
  
• The responsible payment site of the practice (your Finance Department may be able to advise)
  
• The effective start date.

The leaving PCT must ensure:

• that all unused FP10 prescription pads bearing the previous PCT are collected and destroyed in a secure manner
  
• that the practice amend their software to ensure that no further prescriptions are issued bearing the GP's previous PCT.

The joining PCT must:

• Order personalised prescription pads from Astron in respect of the new PCT using the Astron ordering procedure.
