GP Leaving Practice/PCT

The PCT will advise the PPA of a GP leaving their existing practice or the PCT by using the proforma - Notification of GP/Practice Amendments click here to print copy.

Tick the box - GP Leaving Practice/PCT.
Identify the reason for the GP leaving eg resigned,retired,deceased.

The Deletion column should be completed click here for instructions.

The PCT must ensure:

• that all unused FP10 prescription pads are collected and destroyed in a secure manner
  
• that the practice amend their software so that no further prescriptions are issued bearing the details of the GP who is leaving.
