Two forms should be supplied:-

• one by the organisation the hospital unit is leaving
  
• one by the organisation the hospital unit is joining.

The PCT the hospital unit is leaving should:

Tick the box - Change of Organisation.

• The Current Details column should be completed click here for instructions.

The PCT the hospital unit is joining should:

Tick the box - Change of Organisation.

The Amended/New Details column should be completed with the following:

• Name of Hospital Unit
  
• Address (the address which will appear on the prescription form)
  
• Postcode (the postcode of the address identified in the address box)
  
• Telephone Number (the telephone number in case of query)
  
• Hospital Unit Code
  
• Speciality Type
  
• Effective start date.

The leaving PCT must:

• Ensure that all prescriptions bearing the Hospital Unit are collected and destroyed in a secure manner.

The joining PCT must:

• Ensure prescriptions are issued with the relevant details.
