Change of Telephone Number

The PCT will advise the PPA of a change of telephone number for a hospital unit by using the proforma - Notification of Hospital Amendments click here to print copy.

Tick the box - Change of Telephone Number.

• The Current Details column should be completed click here for instructions.

The Amended/New Details Column should be completed with the following details:

• Telephone Number (the new telephone number in case of query)
  
• Effective start date.

The PCT must:

• Ensure that all prescriptions bearing the old telephone number are collected and destroyed in a secure manner
  
• Ensure prescriptions are issued with the new telephone number.
