Change of Address

The PCT will advise the PPA of a change of address for a hospital unit by using the proforma - Notification of Hospital Amendments click here to print copy.

Tick the box - Change of Address.

• The Current Details column should be completed click here for instructions.

The Amended/New Details column should be completed with the following details:

• Address (the address which will appear on the prescription form)
  
• Postcode (the postcode of the address identified in the address box)
  
• Telephone Number (the telephone number in case of query)
  
• Effective start date.

The PCT must:

• Ensure that all prescriptions bearing the old address details are collected and destroyed in a secure manner
  
• Ensure prescriptions are issued with the new address details.
