New Hospital Unit

The PCT will advise the PPA of any new hospital unit required by using the proforma - Notification of Hospital Amendments click here to print copy.

Tick the box - New Hospital Unit.

The Amended/New Details column should be completed with the following details:

• Name of Hospital Unit
  
• Address (the address which will appear on the prescription form)
  
• Postcode (the postcode of the address identified in the address box)
  
• Telephone Number (the telephone number in case of query)
  
• Hospital Unit Code
  
• Speciality Type
  
• Effective start date.

The PCT must:

• Ensure prescriptions are issued with the relevant details.
