The PPA provides Prescribing Budget Input Forms to every PCT before the beginning of each financial year.

These forms record all of the practices and Community Nurse Prescribing Contracts (CNPC) that the PPA have linked to the PCT, along with the current years practice budget.

If any practices included on this document are not the responsibility of your PCT then please cross out - advising, if known, the responsible PCT.

If practices have been omitted then please add the details to the Prescribing Budget Input Form.

Prescribing Budgets should be advised for all practices, any Community Nurse Prescribing Contracts, Walk in Centres and PMS Pilots.

The PCT should complete the following details:

• Prescribing budgets for the next financial year.

The PCT should return this form to the PPA by the date indicated.
