Q. Can I have information on how my claim has been
assessed?
A. You can request information on how your claim has
been assessed by writing to the Pharmacy Reward
Scheme at the address shown, or by faxing them on
0191 203 5730.
Q. Do I have to report the incident to the police before I
can claim my reward?
A. You do have to report the incident to the police.
Q. Must I include the fraudulent prescription form with
the claim form?
A. You should include the relevant prescription form
with any claim unless the police have retained the form.
If you do not have the original, please include a
photocopy if possible.
Q. Does the person making the fraudulent claim have
to be apprehended before I get my reward?
A. No. The reward is independent of any arrest or
conviction.
Q. Must I declare reward payments to the Inland
Revenue or does the PPA do this for me?
A. Yes. Declaration of reward payments to the Inland
Revenue is the responsibility of the claimant.
Q. Is it possible to be eligible for both rewards?
A. Yes, provided the items have not been dispensed. If
the items have been dispensed you will not be eligible
for the basic reward.
Q. If I dispense items and then subsequently find out
that the form was fraudulent, will I still be reimbursed for the items?
A. Yes. You will still be reimbursed by the PPA at the
normal rate.
Q. What happens if another contractor puts in a claim
that relates to the same fraud?
A. If several claims that all meet the assessment
criteria lead to the cessation of one fraud, a Bonus
Reward of 5% of the total savings or £20, 000,
whichever is the smaller, may be shared between the
claimants. The PPA will decide the shares for each
claim or, if this cannot be reasonably be assessed, the
shares will be equal between contractors.
