Your use of this site constitutes your agreement to all terms, conditions and notices contained on the site. If you do not agree with these terms and conditions then you are not authorised to use this Site. |

We must receive payment of the whole of the price for the certificate before your order can be accepted. You must accept these terms & conditions and insert your details into the application form before you can submit your order. Once you have done so we will confirm that your order has been accepted.

USE OF ORDERING SERVICE |

| 10.1 |
This site provides you with information to assist you in determining the suitability of a Certificate for your needs and to order a Certificate, if you choose to. If you do order a Certificate, you confirm that you possess the legal authority to enter into this Agreement (including instructing us to collect any card payments) and to use this Site in accordance with these Terms and Conditions. You agree to be financially responsible for paying for the Certificate you have order. You also confirm that all information supplied by you or members of your household in using this Site is true and accurate. You agree that the ordering service shall be used only to make legitimate orders for you or for another person for whom you are legally authorised to act. |

| 10.2 |
Every effort is made to ensure that certificates are despatched by second class post to your address typically within 8 to 14 days after your order has been confirmed. |

| 10.3 |
It is your responsibility to check that the certificate details are accurate. We are only responsible for issuing Certificates in accordance with your request. Any errors should be notified immediately by e-mail to or by ringing 0845 601 8076. Please have your certificate number ready. |

| 11. |
SALES REFERENCE NUMBER |

You will be given a sales reference number when you complete your order. You must note this reference in case of query regarding your application. Queries should be directed to 0845 601 8076. |

| 12. |
COST OF CERTIFICATES |
| |
|
| 12.1 |
The cost of the Certificate is determined by the Department of Health and is subject to adjustment on 1 April each year. The cost of the Certificate is the price applicable at the time the application is made regardless of the Certificate start date requested. |

| 12.2 |
There is no charge for delivery. |
| |
|
| |
|
| 13 |
CERTIFICATE START DATE |
| |
|
| 13.1 |
Every effort is made to ensure that your Certificate is dispatched typically between 8 to 14 days after your order has been confirmed. |

| 13.2 |
The Start Date that you specify for your Certificate on the Application Form is very important. The Start Date can be back-dated or future-dated by one calendar month from the time of application. Please note that while your Certificate is valid from the Start Date requested, you may not receive it for up to [14 days] after. |

| 13.3 |
Remember to apply for a new Certificate in good time. If you claim you have a valid certificate when you do not, you may be liable to a penalty charge. |

| 14. |
CANCELLATION AND REFUNDS |
| |
|
| 14.1 |
You have the right to cancel this Agreement by written notice to us within 8 days of placing your order. This notice must be provided by e-mail sent to . A full refund will be made using the credit card details that you provided when you made your order. |

| 14.2 |
Except as set out in Clause 14.3 and Clause 14.4, after this 8 day period expires all refunds will be at the PPA's absolute discretion and will only be made where the Certificate has not already been dispatched to you. In no circumstances will a cancellation be allowed or a refund made where a Certificate has already been sent to you, whether you have received it or not. |

| 14.3 |
If you become entitled to financial assistance with your health costs after the cancellation period has expired, you may be eligible for a full or partial refund of a Certificate purchased before you became aware of your entitlement. Please refer in the first instance to leaflet HC11 ‘Are you entitled to help with health costs’, available from main post offices or look at www.doh.gov.uk/nhscharges/hc11.htm. |

| 14.4 |
If you have to pay a prescription charge while you are waiting for your Certificate you can get a refund later. To do this, you must ask your pharmacist or dispensing doctor for form FP57 when you are paying your prescription charge. Form FP57 sets out the details of how to get a refund. Please note that form FP57 can only be issued at the time you pay your prescription charge, you cannot get one later. |

| 14.5 |
If you cancel the Agreement, the obligations contained in Clauses 6 (Copyright and Trademark Notices) and 7 (Liability Disclaimer) shall survive cancellation of the Agreement. |

| 15 |
CHANGES |

| 15.1 |

For a change of name please send us a copy of your marriage certificate/deed poll and your Certificate and we will send you a replacement. |

| 16. |
ELIGIBILITY |
| |
|
| 16.1 |
The Certificates available under the scheme that the PPA administers are for people normally resident in England only. Scotland, Wales and Northern Ireland operate their own schemes. |

|
| 16.2 |
However, a person who is resident in England normally may use their Certificate throughout the United Kingdom. The pharmacist may ask to see the certificate before dispensing the prescription. |
| |
|
| |
|
| 17. |
MODIFICATION OF THESE TERMS AND CONDITIONS |
| |
|
| |
The PPA reserves the right to change the terms, conditions, and notices under which this Site is offered. This right shall not effect the existing terms and conditions accepted by you upon making a purchase using this site prior to the changes being applied. |

| 18. |
GENERAL |
| |

| 18.1 |
The Agreement shall be governed, construed and shall take effect in accordance with the laws of England and Wales and be subject to the exclusive jurisdiction of the English courts |

| 18.2 |
This Agreement shall be concluded in the English language only. |
| |
|
| 18.3 |
The PPA’s performance of this Agreement is subject to existing laws and legal process, and nothing contained in this Agreement is in derogation of the PPA’s right to comply with law enforcement requests or requirements relating to your use of this Site or information provided to or gathered by the PPA with respect to such use. You agree that the PPA may, at its complete discretion, provide details of your use of the Site to any court, regulator, law enforcement agency or other third party which has the power to require such disclosure. |

| 18.4 |
This Agreement constitutes the entire agreement between the customer and the PPA with respect to this Site and it supersedes all prior or contemporaneous communications and proposals, whether electronic, oral, or written, between the customer and the PPA with respect to this Site. A printed version of this Agreement and of any notice given in electronic form shall be admissible in judicial or administrative proceedings based upon or relating to this Agreement to the same extent and subject to the same conditions as other business documents and records originally generated and maintained in printed form. |
