The PPA is continually improving its existing prescribing information systems. As apart of this, a study is being carried out into the future of the PPA paper based prescribing reports and the feasibility of providing more information electronically.
To assist in this study, the on-line questionnaire has been compiled and is being distributed to a sample of report recipients. It would be of great assistance if you could take the time to complete the questionnaire, which should take you approximately 15 minutes. Please provide as much information as possible. If in doubt, include it.
On submission of the questionnaires analysis will be performed to provide input into the study report.

The Prescription Pricing Authority will only use the personal information provided on this form to analyse and thereby enhance PPA reports/electronic systems usage.
No data which can identify an individual will be passed on to any third party or transferred outside of the PPA.
The data entered on this form will be deleted no later than 1st April 2002.
